Dear Parent/Carer,

Flu Vaccination for Children  

This year between September and December the Calderdale Immunisation Team will be visiting your child’s school to offer the Nasal Flu Vaccination.

Please complete the form giving your Consent or Non-Consent and submit it via the platform within 7 days.

If your child is absent on the day of our visit we will be returning to the school to catch up any absentees. 

If, after completing and returning this consent form to school , your child receives the flu vaccination at your GP surgery, please inform the Immunisation Team immediately on 01484 728 956.

If your child becomes wheezy or has their asthma medication increased following completion of the consent form, please inform the Immunisation Team on 01484 728 956.
